PPH-Tampa Now Offers Immigration Physical Exams
Passport Health of Tampa Bay is pleased to announce Dr. Michael A. Cromer, our Medical Director, has been designated as a Civil Surgeon by the United States Citizenship and Immigration Service (USCIS). With this designation, we are now performing Immigration Physicals (Form I-693, Report of Medical Examination and Vaccination Record) at our Tampa Clinic located in the Carrollwood section of Tampa. We are happy to offer you this service to help you progress through the immigration process.
Immigration Physicals are performed most days of the week, and are a two step (visit)process.
First Visit:
You must provide basic information, a passport, sign the required immigration form, and present your vaccine history. We conduct the required physical and mental reviews, tests for immune defenses, Tuberculosis exposure, and other communicable diseases required by USCIS. We stock all USCIS required vaccines and can administer them the same day, or during the follow-up visit.
Second Visit:
You will return to Passport Health of Tampa Bay two days later for the interpretation of tests performed during the first visit, and to receive any further vaccines, medicine, or testing required to complete the I-693 Form.
Upon completion of all USCIS, I-693 Form requirements, you will be given the completed and signed I-693, Report of Medical Examination and Vaccination Records Form, in a sealed envelope. This envelope may not be opened and is to be submitted to the USCIS as part of the application process. An unsealed, identical set of documents will also be given to you for your records.
